Students who wish to enroll at Casa Grande High School must complete the following process. A student may not begin attending classes until all documentation is received and processed.
The following may also be required:

Students who wish to enroll at Casa Grande High School must complete the following process. A student may not begin attending classes until all documentation is received and processed.
- A parent/legal guardian must be present to register the student(s). Please bring photo identification and/or legal documentation.
- Complete a Petaluma City Schools registration form - English or Spanish.
- Check Out/Exit Grades (if enrolling during the school year).
- Transcript from previous school.
- A complete and current record of immunization s from your doctor/clinic, meeting the requirements of the California Department of Health (see below for more information regarding immunizations).
- Polio – 3 doses. One must be given after the 2nd birthday
- DPT/DTap – 3 doses. One must be given after 2nd birthday. TDap Booster – One dose after 7th birthday.
- MMR – 2 doses. One dose must be given after 1st birthday.
- Varicella – 2 doses if entering school from another country or state and have never attended a California school OR provide proof of disease via medical provider notation.
- A clear copy of birth certificate, passport or Visa.
- Proof of residency – Lease/rental agreement or escrow papers OR current utility bill in the name of the parent/guardian. If the student and student’s family are sharing a residence, then a letter must be submitted from the resident listed on the utility bill indicating that the family is living with them.
- If the student is residing with someone other than a parent, a signed caregiver affidavit is necessary. The caregiver must be a qualified relative. This form is available through the Registrar.
- If a student is in Special Education, a copy of the IEP/504 must be provided.
The following may also be required:
- Caregiver affidavit - If the student is residing with someone other than a parent, a signed caregiver affidavit is necessary.
- Shared residency affidavit - A signed affidavit of the resident of the school district verifying that the student and the student's family are residing with him/her at that location. A copy of a current utility bill, in the name of the person signing the affidavit must also be submitted.
- Copy of student's most current report card and/or transcript.
- A copy of the most current IEP, CELDT scores, and/or CAHSEE results.

- Polio - 3 doses. One dose must be given after 2nd birthday.
- DPT/DTaP - 3 doses. One dose must be given after 2nd birthday. TDaP Booster-One dose after 7th birthday.
- MMR - 2 doses. One dose must be given after 1st birthday. If only one dose is given before 1st birthday, a second dose is required. If measles vaccine only, a student must have one MMR on or after 1st birthday.
- Varicella - 2 doses if entering school from another country or state and never attended a California school OR provide proof of disease via medical provider notation.
Immunizations that are recommended:
- Hep B - 3 doses
- Hep A - 2 doses
- Varicella - 2 doses if never had the disease and previously attended a California school.
DIS-ENROLLMENT INFORMATION
- Student(s) must be accompanied by parent/guardian when dis-enrolling. Phone calls will not be acceptable.
- Student(s) must meet with the Registrar to dis-enroll.
- Student(s) must return textbooks/books.
- Student(s) must attend each class to obtain an exit grade. This is a 2 day process (A and B day schedule).
- Student(s) must clear any fees.
